Consent for FloLabs 

Effective Date: July 24, 2025 

FloLabs provides mobile laboratory specimen collection services for physician-ordered tests. Before proceeding, please review the following important information.

What You’re Consenting To 

By booking or accepting this appointment, you acknowledge and agree to the following:

  • You are voluntarily receiving a specimen collection (e.g., blood draw) by a certified phlebotomist.

  • You understand that risks associated with specimen collection may include discomfort, bruising, bleeding, dizziness, or in rare cases, fainting or infection.

  • You understand that specimen collection may cause physical or emotional discomfort, particularly if you have a history of anxiety, trauma, or negative experiences with medical procedures.

  • You understand that multiple attempts may be needed to successfully collect your sample, and in some cases, a recollection may be necessary.

  • You agree to follow any required pre-test instructions (e.g., fasting, medication withholding) provided by your healthcare provider.

  • You acknowledge that FloLabs does not process, interpret, or provide laboratory results. Results will be shared by the testing laboratory or your ordering physician.

Fees and Appointment Policies

  • A service fee applies and is outlined at the time of booking.

  • Additional fees may apply for late cancellations, missed appointments, or if the client is unprepared (e.g., no requisition, unavailable, incorrect instructions followed).

  • Full fee details, including cancellation and no-show policies, are outlined in the FloLabs Terms & Conditions.

If You’re Booking On Behalf of Someone Else

If you are a clinic, healthcare provider, or caregiver booking this appointment for a patient or dependent:

  • You confirm that the individual receiving services is aware of the appointment and has given informed consent to proceed.

  • You confirm you have the authority to consent on their behalf if needed.

  • You acknowledge that FloLabs is relying on your representation and will not be held responsible if appropriate consent was not obtained.

Liability Acknowledgment

FloLabs is committed to safe, professional service. However, by consenting to proceed:

  • You release FloLabs from liability for known and disclosed risks related to specimen collection when performed according to professional standards.

  • You understand that FloLabs is not responsible for delays, errors, or recollections due to third-party labs, couriers, or requisition issues beyond our control.
